Then be sure to obtain two (2) recent and complete bill copies (90 days) for each meter. Once you have all of the information, please remember to fill out the Customer Entry Form (CEF) by clicking on your state in the map below. Once these steps are complete, all that remains is for you to fax in the signed LOA and bill copies to 972.499.8316 or scan and email them to 5linx@diversegy.com.
